Meet the team

Dr Jacobs is originally from the Liverpool area, and completed her initial medical training there.

Dr Jacobs is primarily based at Rush Hill.

When not busy at work, Dr Jacobs spends her time trying to tire our her young children and energetic dog!

Dr Fountain sees patients at both Rush Hill and Weston sites.
Dr Fountain went to medical school at Guys and St Thomas’ Hospitals in London.

He graduated in 1994. Then came to Bath in 1995 and decided to stay!

He enjoys being a GP for the variety it offers and getting to know people.

His main hobbies and interests are Hill Walking, Sport, Cooking, Gardening including his child and wife free allotment!

Clinical Director, Minerva Primary Care Network

After finishing his GP training in 2003, Sam joined Combe Down Surgery in Bath as a partner and has been there for the last 16 years. His clinical interests include GP training, respiratory disease and dermatology.

Sam, who’s the proud father to two teenagers, enjoys playing the drums, loves running, and volunteers at the National Children's Music Camps most years.

Clinically, Sam is based with Combe Down Surgery, where he is a GP partner.  Sam works closely with all the Minerva practices is his role as Clinical Director for Minerva Primary Care Network.

Advanced Clinical Practitioner may be an unfamiliar title, so it is worth explaining what it means. I am a nurse rather than a doctor.

Advanced practice is a level of practice rather than a separate profession, and it means I have trained to assess, diagnose and treat patients in my own right. I can examine you, arrange and interpret investigations, reach a diagnosis, prescribe medication and refer you to hospital or to other services. Where a problem falls outside my area of practice, or where I would value a second opinion, I discuss it with one of the GPs.
I have been a registered nurse since 2006. I spent my first ten years in hospital nursing, in respiratory medicine, gynaecology and intensive care, before moving into general practice in 2016. At my previous practice I worked in the treatment room and went on to run my own clinics for patients with long term conditions including diabetes, heart disease and kidney disease. I joined this practice in 2022, where I have continued that work as a senior nurse and taken on my advanced practice training. Many of you will have met me in those roles.

I am one of several advanced practitioners here. You may also see Kira, Sam, or Clare our Advanced Practice Paramedics, who run our acute home visiting team.
I am clinical lead for the nursing team, supporting three nurses and three healthcare assistants through supervision, training and professional development, which helps us maintain safe and consistent care across the team.

Appointments with me can be booked through our online triage system or with our reception team, who will help you find the right appointment. They are usually a little longer than a standard GP appointment, which reflects my stage of training and allows time for a thorough assessment and a full discussion of any investigations, treatment or referrals.

Sue primarily works at Rush Hill.

Sue also offers the full range of treatment room services, health checks and reviews of patients with asthma and COPD.
